Is it normal for peds office to charge...
To fill out Medical forms for daycare. I was charged 5 bucks a form. I have twins. The forms were not lengthy at all

They tried to charge me $15 but i refused to pay..this was for a school form that I HAD to fill out (shot records) for pre-k then again for K. I think its atrocious to charge $15 to fill out a one page form! !

I made sure to choose a ped who doesn't because I think its disgusting to charge for forms I am mandated to get done for daycare. Camp forms fine, but childcare thats terrible

Our ped charges for non mandatory forms like daycare or camp. For school they do not charge.

When we first started with our ped 6 years ago there was no charge. Now there is a charge to complete forms not presented during a visit.
