Gym reimbursement

Does anyone know how to get gym reimbursement through your insurance company? Tia

I believe there is a form that the ins company provides (sometimes on their website) that you need to fill out and have the gym sign off on with a log of your visits to prove you went the required number of times.

I know my old ins company had the form on the website. The ins co prior to that I would get it from my broker.

Unfortunately I called the insurance company and they said they don't (empire govt) but I have heard of other people at work getting it covered

I also have an empire nys plan I also confirmed that they don't reimburse. It's one of the only insurances I've heard of that doesn't. it sucks.

I don't have a gym membership but I wonder if you could try to submit through a FSA/HSA if you have one?
