I've had it twice - in one instance my dentist fixed it (re-alignment done via retainer), in the other, an orthodontist did it. Can you dentist help you with a recommendation?

Re: TMJ help - Dr. recommendation
My dentist is retiring soon and only deals with basic dentistry. He recommended me to an oral surgeon which didn't work out to well.. I just finished up Invisalign and that didn't help other than straighten my teeth

TMJ help - Dr. recommendation
I have suffered from TMJ most of my life. I wouldn't wish it on anyone. Mine is apparently due to chronic arthritis which has worn away my upper condyle to the point where it is now bone on bone. I have not been too successful in treatment, but I have been to Coram/Selden Dental and they were able to diagnose and offer some temporary treatment options. They have an oral & maxillofacial surgeon there.
Unfortunately, neither my dental insurance nor my health insurance will cover any of it, so I am left to suffer.

I think your best bet is a maxillofacial surgeon/specialist

Re: TMJ help - Dr. recommendation
I've suffered for 6 years and the last two years have been the absolute worst.
mine are also entirely out of place, so its basically bone on bone.
xanax has helped a little but of course you have to be careful with it, so I only take it 2x per week at most. i save it for the most severe days, like when I go for a week with zero relief and im at the point im ready to drop off a cliff.
i sought a tmj specialist and he really said there was nothing he could do for me honestly.
sometimes wearing my retainers i stopped wearing helps a little.
the best thing to try, something it has taken me 6 years to fine... a chiropractor that specializes in tmj, they try natural ways to put the disc back in place and relieve pressure. ive only been for 3 visits so far... it helps for that day at least, temporarily relief for a few hours, but still not enough to make it through life lol. but i'd def recommend that.

I knew a PT who taught me some exercises/movements to do when the pain flared up. They really worked to relieve the pain asap.

I wanted to add it's crazy how much work we have to put into it but massaging the area and doing hot/ice combo, it does help temporarily as well. not a fix and hard to do often but on the worst days i just sit on the couch feeling bad for myself watching tv and going between heating pads and ice packs and massaging the jaw and ear areas!

Yes. Dr Richter from patchogue family dental. They are a father/daughter team. His other daughter is a dental assistant.
I have the same issue. I'm in braces to correct the problem. I have had them for 14 months and I'm Almost done. My jaw did not line up and I was a chronic teeth grinder at night.
They really take their time with you. Really try and find what works for you.
The whole office staff is the nicest staff that I don't mind going to the dentist.

Re: TMJ help - Dr. recommendation
An Oral surgeon - however all they suggested for me is to wear a nightguard because I clench my teeth. I def have pain radiating to my ear though. Nightguard doesnt really fix the problem.

I hear you. Made and cancelled 2 appointments so far but I'm nearing complete desperation. I've had my jaw lock open 7 times since August.
