How to ask for medical records
I'm switching practices and need copies of my chart to show the new one. How do I call and ask for them? I'm so nervous, like I'm breaking up with them

How to ask for medical records
Just call and ask. Often you will have to pick them up and even pay a small fee for the copies, like 10 cents for each page.
You might always say you are going for a second opinion and need the chart.

How to ask for medical records
Just call and ask someone in billing. I recently did this and it took awhile because they send it out to be copied. It then got lost in the mail, total nightmare, and I ended up having to cancel my appt b/c I didn't have the records. So what I'm saying is, just call and do it, sooner than later if you need them for another dr. appt.

Sometimes practices have forms you can send(or they can send) to request the new practice receive your chart.
