It goes inside the fireplace and is sealed and blows out hot air - heats the whole house. We don't even have our heat on and & it is perfectly warm. My DH got it through a contact of his but they sell them at fireplace stores. Here's a pic:

We bought one a couple of months ago. Ours is a wood pellet insert, it burns wood that is compressed into small pellets. You can also get some that are gas or propane, or that burn actual wood.
Our house is 2965 square feet and it is a split level. If the house was a ranch I think it would heat the entire house, but because it's a strange layout, I'm not *too* happy with it...it heats the downstairs and the second level, but the top two levels are kind of chilly. We put a fan (tiny fan that makes no noise) in the doorway leading upstairs to attempt to blow the air that way.
I think it's making a difference in our heating bills, because we have electric heat and were paying upwards of $600+ a month last winter forr heating bills. So far we have only had to turn on the electric heat in my bedroom.

We have a wood burning fireplace but I believe they make them for gas models too.

For anyone looking to maximize their heat output in a regular fireplace. They make blowers for gas & wood fireplaces. We just found out about the gas blower this year. WoodlandDirect.com has some great products!

My DH would know better, but I would say approx. $2k. But think of the $ you save on gas heating bills!
