Both formula around 13-14 months old-we started the milk transition at 12 months old and it took a little bit to use up our supply-#2 also had protein allergy.
Bottle #1-18 months
Bottle #2-15 months

A bottle- we're still on it at 15 months, but really don't need it. She drinks out of a sippy cup mostly.

12 1/2 months. We did both at the same time, but couldn't do cold turkey to milk b/c we had to do a milk challenge first.

Formula, around a year. Though DS is a year old, and I am starting him on milk tomorrow. Little by little though, he has had consitpation problems in the past so I don't wan tot switch all at once.
His doctor said she would like to see him completely off bottles by 18 months. We've reduced one per day at this point (he drinks out of a cup then).
