Disability forms Q'

When do you get your doc to fill in the forms - when you have delivered and are at the hospital?

I have no HR department so I have to complete these myself and I dont know when to get this part completed.

I dropped them by the office a few days after I was home. I suppose you can ask them to fill them out when they visit you after the birth, but I wouldnt' want to chance them getting lost.
