In my old office the girl that worked there just had surgery to remove a brain tumor.
I was smelling a weird smell one day and no one else smelled it. I joked and said "oh, I probably have a brain tumor" totally forgetting about the girl in the office. Someone was all "god forbid!!!"

I don't want to get too specific and accidentally identify the person or her relatives, so I'm not going to use real names.
We had a family member (now deceased) who some others had referred to (behind her back) as the name of a particular villain. Let's just say it was the Wicked Witch of the West (except it wasn't...it was much worse and more specific). They didn't call her this name all the name or anything; I probably heard it a couple of times.
Anyway, when I was a little kid, I had no idea who this villain was. I thought it was just a nickname for this woman. One day, I called her that in a very happenstance way. I was just like "Hey (insert villain's name)." My family was completely horrified.
ETA: I don't even remember what happened after that. I was super young. I think someone just ushered me away into another room or something.
