Freaking out...low ovarian reserve - SIRM?
I just got some devastating news. My ovarian reserve is a 1 out of 20, the lowest possible. I feel like I am going to throw up. I was pregnant 6 months ago (mc at 7 wks) so not sure how it could be so low. I just turned 39.
I am desperate. I live on LI, but I am willing to travel into NYC if it gives me a better shot. I know a lot of you have had success with SIRM when you've had no success otherwise, so I just made an appt with Dr Tortiello for November.
Someone please tell me I have a chance.

Do you know Exactly what test was taken that gave you a result of a 1 out of 20? I dont know any Ovarian Reserve test that go as high as 20. If its FSH; the lower number the better; and you have a 1.
And AMH levels; usually go up to 4; not 20, so a 1 is not extremely terrible.
Do you happen to know what your FSH level is at? That test is usually taken on CD 3 of your cycle.

I don't know the test. The nurse told me 1 was the worst, the lowest...and it's what I scored. My dr will call tomorrow to tell me my options.
I don't know my FSH. I had my AMH levels done a year or so ago and they were .6 or .4 I think....they were bad too.
I am hysterical. I am willing to do anything, go anywhere...we are just at a loss here.

Same questions. What test did you have exactly? Ways to detect ovarian reserve... Amh levels AFC on cd3 FSH levels.

Maybe they measured your AFC. Did they do a sono to get this result? Bc you want around 20.

It was a blood test. It was around Day 3. Could it have been that test?

I don't have low ovarian reserve but I did have success (currently preg via IVF) at SIRM after MANY failed cycles elsewhere. I live on LI too and SIRM is not convenient but I made it work bc I knew it was my only chance. SIRM worked miracles for me since I was about to give up hope.
Did you ask your AMH number and your FSH number? My FSH is a little high.

Your amh is just an indicator of reserve but not an indicator of quality so you may have some good eggs in there still. My amh was originally .28 when I started TTC. I went gluten free, did acupuncture and took supplements and it went up to .78. After my last IVF, it went down to .38. It can fluctuate and one thing all REs told me was do not obsess about the numbers. You can have good and bad months with hormone levels, especially as you get older. There were months my AFC was a 3 had some that were 10. Some months my FSH was sky high and others it was much less. All these numbers show us is how easy it will be for you to get pregnant and it will be difficult with those base levels but it doesn't mean that it's impossible. I wonder what test this was!! I'm so curious.

They use sonogram to measure AFC so I have no idea what test this is. This nurse shouldn't be freaking you out. See what the Dr recommends.
Who do you see now?? Wasn't it Braverman? Or was it Kofinas?

Kofinas the RE or perinatologist? If it's the perinatologist, then you may have to start seeing a RE to begin testing, especially if your numbers are low. Being that you got pregnant naturally, you may have a good chance still.

It's the peri. I had 3 failed FET'a at RS of NY. I have an appt for a consult w San Roman in Oct & Dr T at SIRM in Nov.

Yea, I remember when you were at RS. :/ I think it's time to pull out the big guns. See what Kofinas thinks about the results bc he is very good at what he does. His opinion is well respected among REs in NY. Dr. T and Dr. Wang are awesome and pretty intricate with treatment so you can't go wrong going there. Did Kofinas do an immune panel on you?

I think I might know about the test you had, except dr. Braverman did this test when I was his patient. It was done on 3 rd cycle day and blood was taken. It was also called ovarian assessment reserve or something like that. It gives you a number indicating how many eggs they hope to retrieve through ivf based on Fsh, prolactin, and other measures. It had scores similar to what you mentioned and it was broken up into poor, fair, good, and excellent. My number was a 9 and it came out to a fair chance at a successful ivf with the hope of retrieving 9 eggs. When I actually had IVF about 6 months after that test was done, they retrieved 7 mature eggs. I had to do 6 iuis before I could advance to IVF because of insurance. Anything over 15 was considered an excellent chance at IVF with the hope of retrieving 15 or more eggs. Try not to dwell so much on specific numbers. I have known some people who took this test I mentioned with lower numbers and a less than fair chance of retrieving less eggs and yet they went on to have healthy babies. Best of luck with everything.
I know kofinas and braverman usually work together with regard to any immune issues. I think they do similar testing. I would suggest u stick with Kofinas and see what he has to say. Also, Dr. T from SIRM used to work with Dr. Braverman some years ago. That's good that u have other appointments. Try to bring copies of this test as well as other tests. Good luck!

I'm in same boat as you. My Amh is .16 and my ovariAn reserve is low. I have an appt with Dr. T in November also!!!! I'm 40 not too far ahead of you. In my opinion SIRM is my last chance . Hang in hot here!!! Xo

Yes, I had the immune panel & he diagnosed me w a genetic & acquired thrombophilia, PAI -1 4G/5G. We had one DQ Alpha matches but he was less concerned about those. I'm on Metformin 2x/day.

My amh was .26 or .24 (can't recall which) during my last IVF cycle. I was 39 at the time. I have a 3 week old DS as a result of that cycle. There is absolutely hope

What insurance do you have? I have Empire UHC. I was under the impression that dr t was out of network for me but when I contacted medical secretary she said he takes my insurance - but first visit not billed as infertility. I'm so confused

if insurance does not cover certain codes or issues putting your consult in and billing for a different issues would allow at least that visit to be covered. so, if your coverage wont allow infertility but will allow fibroids to be covered, and you "discussed" fibroids the office can bill for that code. from what I have been told

When I made my appt, the receptionist asked if the word Empire was on my card when I told her I has NY SHIP...she said if it's Empire, you're covered. I have Empire.... Do you?
