My hairdresser usually does everything (20%tip)
Otherwise $3-5 for wash tip.

wow, I must be really cheap! I tip my hairstylist 20% and the girl who washes my hair $1!
I don't go to an overly fancy salon (wash/cut/blowdry is $40)

as a stylist myself...
i know the assistants really appreciate tips...for a simple wash 2-3 dollars..
if doing highlight washing toning or coloring 5 is very nice!!!
