There are 3 very reputable inpatient rehab facilities in the area, BUT, he is on dialysis, and most dialysis centers in our area, the philly suburbs, won't take a dialysis patient on a temporary basis, that's why we placed him in a facility close to where I live because that's where his permanent dialysis center is.
With that in mind, I'm calling the case worker from the hospital today, and telling her that she is going to have to place him in one of those three facilities TODAY AND arrange for dialysis on a temporary basis AND arrange for his transportation.
If she doesn't comply, or resists, I'm heading over to the hopsital in person, and giving her, and everyone I can get my hands on, an earful.
Also ask her about what other services your father is eligible for. There are many many services for the elderly that most people aren't aware of... ex: transportation, at home care aid, etc. Also, doesn't the rehab facility have any sort of transportation available? Meaning if he was transferred to a different place, isn't there some sort of transportation options to get him to dialysis?
Just to expand on thsi is it possible to get this care at home? That might be a better option and he would be more comfortable

I called the Town of Southampton Senior Services for information for my mother. I'm sure if you type in your town and senior services in yahoo, something should come up with a number to call. Give them a call to talk about your options and/or if they can redirect you to someone who can help for your situation.

He's over 65, and on Medicaid, but fortunately, or unfortunately, however you look at it, he receives a large pension every month from his former firm, so he doesn't qualify for most of these services, which leaves me scrambling to get everything arranged by myself.

I just talked to him - he had to argue again with the "nurse" about his insuling. They were about to give him an amount that would have sent him into severe, severe shock and when he told the nurse, the nurse said, well, the insulin raises your blood sugar. No YOU FREAKIN' IDIOT, if you give him too much, his blood sugar will drop, he'll become incoherent, and go into shock.
He also asked for a shower, didn't get one, and they haven't given him the rest of his medications. And, although there is "supposed" to be a doctor on the floor, there ISN'T. The typical nursing home phantom doctor. And we wonder why there are sixty gazillion lawsuits against these kind of places? Just before I left my old firm, we had a case against a nursing home. The doctor SAID he visited the patient everyday and gave him a physical but the records showed he didn't come even ONCE. They ignored the patient's bed sore, pills got lodged inside the wound, and it got so bad that she had to be transferred to a hospital for immediate surgery. I'm starting to think this is the norm at these kind of places.
I'm so disgusted.

Honestly i'd forget looking online and i'd call the senior services in your town. I've talked to so many people to get my mom help and they've ALL been so kind and helpful!!! Plus they know exactly what works and what doesn't so it would be much easier for you!

I know this sucks & you've reached your limit. I would pick up the phone once you get the number. I agree the caseworker should be doing more for you.
If you need to look on the brightside, you could be making those phone calls at your old job.

I'm so sorry you have so much to deal with. It's horrible trying to find just the right place for any of our elderly loved ones. My boss had a major stroke 9 years ago and he was paralyzed on one side and even with the his family has he got 1/2 azz care at best for awhile. I know you will find the right solution for your Dad. Hang in there.
ETA: He's not even my Father and I pitched a beyotch many times on his behalf. Well he has done so much for me and my girls...
