Jury Questionaire question - regarding maiden name and old address- esp for the attorneys or those who know the court system
My mom emailed me a little while ago to tell me they received a jury questionaire in my maiden name to their home which she accidently opened since it was in my maiden name (I haven't lived there in over 12 years and my driver's license hasn't had that address for about 8-10 years - have had 3 other addresses SINCE then both in MI and in NY). She is insisting that I have to fill it out and send back it in.
I legally changed my name when I got married back in April 2004 and I obviously haven't lived at that address for a very long time. I don't want to deal with jury duty (I believe she said it was for Eastern district of NY) right now esp b.c I have a 21 month old that I am the sole care provider for and I'm almost 28 weeks pregnant.
My question is legally do I have to respond since it is in my maiden name at my parents address that is not my NYS legal name or address? It is a questionaire not a summons -but I don't want to have a problem but I'm wondering if by law I have to respond to it. She said there is no respond by date.

I had the same problem. I called them up and told them that is no longer my legal name and I no longer live at that address (it was even in a different County so it was a completely different court system) and they took me off the list.
I felt better calling- just to clear it up- rather than just ignoring it...

I'd call them and tell them. You would get out of it b/c you are your DD's primary caregiver anyway.
Is it Federal Court? They aren't as caught up.
I just got one too....but in my correct name.

I had the same thing happened. I ignored it since it was sent to my parents' house in my maiden name. I ended up getting two more- the last one demanding I respond. So I filled it out & wrote in my married name & new address & sent it back. That was nearly a year ago & I haven't heard anything since so I guess that satisfied them.

Okay so I will just fill it out - I just don't want to have to deal with brooklyn if I don't need to and I think they will hit me again in a few months b.c my driver's license it up in Sept so I know I will get one then b.c I always seem to get one after I renew my DL. Better safe than sorry.

this happened to me too. i had to fill out a form and send copies of my drivers licence and 2 utility bills with my married name and address.
i moved out of state, so it might be a bit different....

The maiden name doesn't matter since you are still the same person even if you changed your last name so if you still lived there, you'd be required to appear or furnish a reason why should be excused. But I would respond with proof that you live elsewhere.. I mean, you could not respond since your parents could have sold the house and the current owner might just throw them out but since you do have access to the mail, I'd call or send the proof so they could take you off the list.
