are you considered high risk?

i just found out that am I high risk due to my blood clotting issue - PAI-1 4G/5G polymorphism. i have been on lovenox for the last few months and was on it during my IVF cycle prior to ER and ever since ET. i have to go for bloodwork this week to make sure the lovenox has been effective.

i am sort of relieved that i'll get a lot of monitoring during my pregnancy although i really wasn't expecting to be high risk. i know a lot of people here see Dr. Kofinas. Where is he located? any other high risk recs? im considering Great Neck ob/gyn and Dr. Rifkin since they do high risk also.

Thanks!

Re: Lovenox / High-Risk Ladies

you will be considered high risk b/c of the clotting issue b/c clots can form in the placenta which the lovenox will help but if you do get those clots in the placenta they need to be monitored to make sure the placenta is healthy enough to suppor the baby...i have clots and take lovenox daily.

i see dr kofinas and love him to death!! he's located on stewart ave in garden city 516-832-0300...his office staff is so incredibly friendly too!

good luck!

Re: Lovenox / High-Risk Ladies

I chose to see Dr. Kofina's b/c everyone else who was on Lovenox was seeing him too. After everything we'd been through, I wasn't taking ANY chances w/ this baby.

He recommended Dr. Pillitteri to me, so, that's the OB I see as well. I feel like I'm in very good hands.

(Yes, very spoiled that I get to see this baby every 2 weeks, sometimes, every week!)

I have the blood clotting issue & NK cells. I also now have placenta previa which they're monitoring closely. They don't miss anything!!!!! Chat Icon

Good luck!

i also had elevated NK cells although according to my recent immune panel after intralipids so far everything is in the normal range (for now).

so a high-risk doctor monitors but your ob/gyn still delivers? Chat Icon Chat Icon

Re: Lovenox / High-Risk Ladies

I am also on lovenox for a blood clotting issue and had/have elevated NKT Cells but my levels are also fine after intralipids.

Dr. B refered me to Dr. Pillitteri who is a high risk doctor. They both consider me high risk because of the clotting issues. Dr. Pillitteri recommended I see Dr. Kofinas. So Dr. Pillitteri will deliver and I see him once a month but Dr. Kofinas monitors me and I see him (or the sono tech) every two weeks.

I love going to Dr. Kofinas' office because I know I will see the baby every two weeks and their machines are awesome! I also really like my sono tech Diane. She sees me every time I go.

I would highly recommend the Pillitteri/Kofinas combo...you can't go wrong!!

I see Dr Kofinas / Dr Pillitteri too! I have 2 copies of MTHFR and some other assorted clotting issues both immunological and aquired. I know that if it wasn't for Dr. Kofinas and putting me on Lovenox my twins would not have made it. I saw him for the first time at 8 1/2 weeks and both of each of their placentas were half dead already due to clots and I have a huge subchorionic bleed and some previa. I am monitored VERY closely and my babies are doing well so far! I highly recommend them both...they are both GREAT!!
(AND I travel over an hour to see Dr Kofinas and 45 mins to see Dr Pillitteri...they are SOOOOO worth it!!)

I was also recommended by Dr. Braverman to see dr. Kofinas and absolutly love that office. I will warn you that meeting him for the first time you might leave there very scard and sometimes his personality is not the best. BUT he and Dr. Nguyen are amazing Drs. And I could not thank them enough for what they have done for us so far. All of the staff are great too. I go to univeristy plaza right next door for my OB and will deliver at winthrop.

I totally agree about Dr. Kofinas and your first visit...I was terrified but I have learned he is just an amazing doctor who will take every step possible for you to have a nice healthy baby. And as we have gone along he has loosened up with us a little and I feel so comfortable with him. I have my far share of issues this pregnancy and after that first visit with Dr Kofinas I have never once been scared about my babies!!!!

Re: Lovenox / High-Risk Ladies

I live about 5 minutes from NSUH and LIJ as well, but I do travel out to Dr. Pilliterri. He is worth it! He sees me on Saturdays so there is almost never a wait and he lets Dr. Kofinas do some of the prenatal stuff so I'm not trekking out there so often. He is just amazing.

Dr. Kofinas is amazing as well. The PP were correct that the first visit you may leave scared out of your mind, but I loved the fact that he told me that he considered anything other than a full term delivery of a healthy baby a personal failure on his part!

Do I still wish I was delivering at NS? Of course I do, but the life and health of my baby is worth the inconvenience.

I have hetero MTHFR, NK cells, and high cytokines. I was on prednisone for 18 weeks and am now on 40 mg lovenox 2x a day and baby aspirin, and vitamin D3, + extra Folic Acid.

Best of luck, but I can say that they are both worth the driving, even in this weather!
