DS is 17 months and in my diaper bag (which actually is a back pack), I have a change of outfit, sippy cup, diapers, wipes, snacks & small toys or books.

I usually just take one of my larger purses...
ziploc of wipes, 2 diapers, (emergency pacifier), sippy with water and a small container of gold fish crakers and a bib, and sometimes I throw in a mini bagel with cream cheese for emergency situations....
I'll never forget when the Roosevelt Island tram was stranded and the babies on it were starving....that's my biggest fear being in stranded with no food for the kids...

Diapers, wipes, sippy, juice box for Sarah, snacks (easier to bring my own since we have to deal with food allergy), a travel aquadoodle set for each, epi-pen
For me:
wallet, keys, phone, gum, small pack of tissues, purell, pen

I just bring shove my wallet into a canvas shopping bag, I bring a diapees and wipees case, (so thats diapers and wipes) a juice box or a sippy cup, some snacks, some toy cars (preferably a neenaw car) and thats pretty much it!

I use a skip hop bag and put diapers, wipes, tissues, change of clothes, snacks, sippy cup, bottle of water, poopy bags, birp cloth (not for birping, just to clean a spill or something when we are out), small toys and my wristlet with wallet it in.

Diapers, wipes, a changing pad, bags for dirty diapers, disposable bibs, pacifier in a holder, sippy cup, a snack or 2 (depending on how long we'll be out), a comb and a hair rubberband or clip, and a few small toys.

little toys and some books
Diaper Rash Cream
Disposable Bags for the poopy diapers
Towels and a plastic bag (this is for the car because he sometimes throws up)
And then I throw in some of my stuff like keys, wallet, and phone etc etc...

Depending on where we're going I'll throw in a small toy or two or a book, but usually I keep those in the car or in the bottom of the stroller.

Hmm, an outfit change since she's a piggy, a toddler spoon and fork, a few diapers, some wipes, tons of snacks and a cup. Then my stuff.

A bare minimum here, and I have 2 kiddies. A diaper (yes, just 1), wipes, 2 sippy cups (even though my oldest should not be having one at 3.5) and some diaper disposal sacks.
