passport name change
How do I change my name on my passport? Is there a website I can go to to print out the paperwork?

Re: passport name change
Or you can just stop at the post office and they will give you what you need. I just sent mine out a week ago. You need to send the form, 2 passport photos, your old passport and your original marriage license. FM me if you need more details.

You can send the copy of your marriage license -- you don't need the original.

Re: passport name change
I went to the PO and filled out a new application. I had to change the address on the passport as well as my last name.
I had a new photo taken, completed the form, submitted it with my old passport and marriage license on the Friday after T-Day and I got my new passport with my old one back (I asked for it to be returned) and my marriage license within 3 weeks - I had it before Xmas week.

Re: passport name change
It was very easy, I just mailed in my marriage license, it can't be just a regular copy, they said it had to be the original or a certified copy and sent that in with my passport and I got it back in two weeks. Because mine wasn't expiring for awhile, I basically got my old one back with a typed note on the first page saying my name was changed legally to so and so. They said when I renew, I will get one with new pictures.

Re: passport name change
I wouldn't and won't feel comfortable mailing my ORIGINAL marriage license to the government... is everyone sure it has to be the original?

Re: passport name change
Yes, it must be the original or a notarized copy. A regular copy will not suffice.
Yes, you do get it back, along with your old passport which they invalidate by punching a hole through the cover.
If you submit the forms for the name change and your other passport is less than a year old, you do not have to pay a fee.
