Ped well visits

Do you have to fill out any paperwork on your DC's milestones or their developments for the well visits?

I didnt have to yet, I thought maybe it would be coming, but I just went for his 10 month appt and still nothing. I guess my office doesnt do it.

Just wondering if its less common than I think. I know a lot of people who have had to fill out paperwork each well visit on their DC and their developments, rolling over, crawling, reaching, standing, etc...

I think that started at the 12 month visit, but I'm not positive.

We never have. Our ped asks a few questions each time though. And she checks certain things. Like she'll ask "Can he transfer an object from one hand to the other?" or "Is he babbling?" and she'll check how well he can sit, bear weight on his legs, etc.

Our ped and his PA asked us those questions during 6m checkup, but its never anything I've had to fill out

The PA asks us other milestone questions before the visit during other checkups

My ped office gave me a few forms that asked if he rolled over or did this or that... there were I think 25 questions on it... simple yes / no answers. they would only tell you to go to a certain number...and if there were more then 3 nos they I guess would talk about it at the appt. But once that sheet was complete of all yes, they havnt given me a sheet since. But, I still need to fill out insurance papers each time and nothing has changed...that's getting annoying now LOL

yes the 12 mo, 18mo and 24mo I filled out some papers .. its the guideline they use for Autism and to check and see if they child has tendancies

yes, i filled it out for the first few visits, and now Dr is filling them out for me!

Never had to fill anything out but the nurse asks us developmental questions before the Dr comes in - like does she reach for familar objects, does she coo, etc etc - and she records it in the computer.

Yes, I fill something out every 2 months for the first year....then after that, at the 18, and 24 month etc visits.

Nope - mine doesn't in fact he barely asks us any questions at all. I kind of asked him once about it and his response was that he watches the child and the questions he asks them, and what he observes are generally enough - and then some parents have their own questions - and he takes all those things into account. Honestly it would be nice for a check list though b.c you will know straight off if they are on track or not.

Ours was either 12 or 18 months. Other than that she would just ask me is he babbling, is he pulling up etc.

My Ped. does the same and than tells me what I should look for during the following month.
