Re: reasons for a c-section?

I know the risk of a VBAC is rupturing the uterus but there is less than 1% chance of that happening. C-section's also have risks so for me it's worth the chance and as of now I am going with a VBAC unless my doctor tells me otherwise.

I had an unplanned c-section - baby was too big, no descending and I didn't progress past 7 cms total (only 2 cm in 8 hours) and other issues - but they worried about shoulder dystocia b.c she would have been high risk for it. I will be another section for my next child as I am not a good candidate for VBAC - nor do I want to take that risk.

I just wanted to mention something stated above - (this is not directed at Kierasmom - just using something she said) - For many people 1 section does equal another section - the risk of rupture is greater than 1% for many people - 1% is only the risk if you are in the optimal category for VBAC. There are a select few drs out there who are willing to try for VBAC if the conditions are right - but most drs won't b.c of the risks for the baby. So some people aren't automatically another section but for most they usually are.

I heard that small people sometimes get them from being too small to deliver because the baby can get trapped on the way out. I don't know how true this is though.

That happened with me. I am short and was overdue with my first with no progress, AND the baby was big. They did a c-section as they thought he would never fit through.

Re: reasons for a c-section?

My boys shared the same placenta and my doctor said that he would only do a c-section with these type of twins. Apparently their is an issue that can occur that one baby can tear the umbilical chord of the other one on his way out. They actually wound up going earlier because I was having issues with TTTS (which only happens with identical twins who share the same placenta).

If i do have another I would welcome the idea of having a VBAC, but I will go with what the doctor thinks is best for me.

There is no reason to be scared. I was terrified and had a great experience and great recovery. Every body is different! Just know that I promise you 1 thing, no matter the deliever or the length of recovery it will all be worth it when you hold your DC!

Re: reasons for a c-section?

I had a C-Section because I developed preeclampsia and my platelets were dropping. They originally induced me but they said I would most likely have a C-Section anyway so instead of waiting for my platelets to drop anymore we did it right away.

i might need one because i have a low-lying placenta... another reason is if the baby is too big...

ok i definitely heard of the "bab is too big" reason ...thank uChat Icon

Usually the baby being big is not a reason. I had a 9lb 10 oz baby without an episiotomy without any problems. I had the baby out in 3 pushes.

So glad you posted this. The "big baby" thing is actually very controversial. My cousin had a scheduled c-section her "big baby" that came out a whopping 6 lbs 8 0z -- oops!

ACOG (American College of Gynocolgy) has actually come out with a statement against scheduled C's for suspected "big babies."

Most women can birth whatever size baby they can grow. Problems come from mal-positioning -- a badly positioned 7 lbs baby can cause more problems than a well-positioned 10 pounder.

Aside from rare medical conditions for a mother, the main reason I can think of for a scheduled c-section is placenta previa -- the placenta covers the cervix and labor and dialation will lead to hemmorraging in the mother.

Most people schedule C's for breeches now because few doctors have the skill to deliver them anymore since C's have become so much safer, most people will opt for that instead.

Are you talking about mothers without low transverse scars? Because here's some information from ICAN on the safety of VBACs and it seems like a good option:
The rate of reported uterine rupture in planned VBAC with a low transverse scar has ranged from 09% to .22%. This risk is thirty times lower than any other unpredictable childbirth emergency such as acute fetal distress, premature separation of the placenta and prolapsed umbilical cord. A 1994 study based on 5733 planned labours after one or more cesareans reported a rupture rate of .8% with no maternal deaths related to uterine rupture (Guide to Effective Care in Pregnancy and Childbirth 1992; Obstetric Gynecology 1994).
Maternal morbidity rates are consistently and substantially lower for women who plan a VBAC - 2%-23% - than for women who have an elective repeat cesarean - 11%-38% (Guide to Effective Care in Pregnancy and Childbirth 1992).

Here's a link to ICAN's website for anyone who is interested in information on VBACs ICAN

As far as always having a c-section, my doctor is all for doing VBAC's. I would suggest talking to your doctor beforehand and find out what their opinion of it is. I know the risk of a VBAC is rupturing the uterus but there is less than 1% chance of that happening. C-section's also have risks so for me it's worth the chance and as of now I am going with a VBAC unless my doctor tells me otherwise.

I'm so happy to hear that your trying to VBAC!! Chat Icon Good Luck, let me know if you have any questions!

Thank you for your support. I'm hoping to do the VBAC but we'll see if the baby allows it!! This baby is sitting up very high, which was one of the problems I had with DD so I'm concerned that if the baby doesn't move down the VBAC is going to be ruled out as an option. But as long as the doctor does not think I'll have a problem I'm going with it. I think a lot of women are misinformed by their doctors regarding VBAC's and I personally feel that a lot of doctors jump too quickly for needing a second c-section. I am very happy that my doctor is willing to try.

Re: reasons for a c-section?

In my situation my Dr. could "tell" at my 38 week appointment that DS would be a section because of his position...he was extremely high up and not dropping at all! The morning of my 39 week appointment my water was breaking (trickles) went to the Dr. (they wanted to examine me before the hospital) and was told they could induce me, but were 99% sure it would result in a section anyway because of DS position (still very high and did not drop at all) so they sent me to the hospital for a "scheduled section" that day.

The only thing I don't understand about this reason is that at 38 weeks many babies are high and don't drop...actually many don't drop until active labor has begun...I guess I'm just confused by this. I understand that when your water broke and the positioning was too high....I just can't understand a doctor telling a patient that it would most def. be a section b/c of position at 38 weeks...

I totally agree -- I can't believe that any doctor would predict a C because a baby hadn't dropped at 38 weeks -- that's ludicrous. DS didn't drop even when I was in active labor -- he was still +2 when I started pushing -- took me 2 1/2 hours, but I got that little bugger out all on my own.

Re: reasons for a c-section?

Lots of reasons for c-section.

I had horrible "back door" issues during the PG (aka hemorrhoids). So bad that I had an outpatient procedure at 24 wks PG and avoided another one at 29 wks PG. For me, a c-section was decided for a "speedy" recovery. A vaginal delivery would have been horrible with weeks of recovery.

It ended up being so bad that we scheduled my c-section at 39 weeks. Unfortunatelly, 2 days prior to it, I had a bad "attach". I barely dragged myself to the hospital. They even had a colon/rectal surgeon come see me in the recovery room after I had DS. We also had considered having the colon/rectal surgeon do the surgery at the same time as the c-section but we did not have time to prepare for it and my OB was not too hot about doing it at the same time.

In regards to the VBAC, a lot of dr. don't do them after a c-section. This is an option but most dr. don't want the "liability" of it. My OB told me she would not do it.

Re: reasons for a c-section?

I didn't plan to have a c/s - the cervadil did nothing - we did it twice - 10 hrs each, and saw no need for the pitocin - I wasn't dilated, effacde, and he was pretty much under my rib cage and not coming down....

Re: reasons for a c-section?

also preeclampsia......
baby being inside too long (apparently, the placenta doesn't work as well after 41 weeks or so, and can stress the baby)
tons of reasons.... usually it's becuase of baby &/or mommy stress......

You bring up a good point, which I always mention, but not in this post because it wasn't about VBAC. In order to have a VBAC, there are many conditions to meet. You need to go into labor on your own, you can't be induced, you can't have a version, I was not allowed to go past my EDD, etc. My OB said VBACs are not considered for patients who had prior stalled labors or any other problems that led to the first c/s.

My OB fully supported my VBAC because the SOLE reason I had c/s #1 was because of a breech baby. I never went into labor and there were no issues or reasons that a vaginal birth wasn't possible for me. I fully understand the reasons for going c/s again with #2, I just wish more doctors would entertain the idea because it really was a no issue birth for my DD (exactly 2 years ago today! Chat Icon )
