No, I don't get offended - nor do I get offended if someone doesn't say it.
Actually, since I am a succession-sneezer I sometimes get annoyed that I have to say Thank You a half dozen times between sneezes. I honestly wish the whole "bless you" thing would just stop - it's really rather silly.

No. And I actually never hear it from my family. Their religion doesn't allow this to be said...for the "silliness" of its origin.
