Does anyone know how to obtain these when the practice you were vaccinated at no long exists?

maybe try department of health. I think a lot of records are computerized. or a hospital?

I think you can also take a blood test that proves you were immunized.

You can get a titers (blood test) for just about everything that needs proof of vaccination.
