I think it depends on the position of the baby and how close to the opening it is. DS never really dropped - so they had to go far. in the office wasn't bad, but I will say that during labor was bad! But there is something they can do up there to stimulate a drop and I think that's what they were doing for me.
as for the timing. My office doesn't start until 38 to reduce the risk of bringing in an outside infection. With gloves and sterilization there is nothing to worry about, but I don't argue not getting them until so late.

This is why my Doctors don't do them until 37-38 weeks. Most of the time even if you are dilated nothing will happen for a few weeks if you are at 35 weeks.
