when do you have to submit your disability forms?
im 35 weeks. how does it work with the disability forms? i asked HR about it and they keep saying they'll give the forms to me closer to my due date but havent gotten anything yet. do they fill them out and then i just turn them in to my dr?

Hmmm..i guess every company is different.
with mine, we have an outside management firm that manages disability and short term dis. continuation. I was told to start the paper work no later than 30 days before my expected date of leave. I met wit h/r 2 weeks ago and they gave me the forms for this company. I already submitted what i had to..figured I might as well b/c i've been told it takes a while. My dr. filled out their part and i already have my claim #s. Now all i have to do is call them when the baby is born b/c thats the official day im disabled.
your 35 weeks..how much closer do they want you to be??
