Hi all,
Please help! I went to a maternity workshop and I think I know what I have to do..the problem is the paperwork is confusing me..does anyone know anything about the op198 form...regarding filling it out?

Which form is that? The SLOAC one?
What's your payroll secretary like? I sat down with her and did all the paperwork- I ended up having to go out on leave earlier than planned- but it was much easier than trying to figure it out myself.

No it's the basic 6 week maternity leave form..too late to do with secretary I already started my leave. She wasn't too open for that anyway.

I wish I could help... but I won't be going to the workshop for another month or so...
I just wanted to say that my payroll secretary tells everyone that she hates pregnant people. Yup... feels good to be working with her.

I wish I could help too but my payroll isn't helpful at all... I took the 6 week and she made me full out only the FMLA form and forgot to tell payroll I can back sooooo now I haven't gotten paid this pay period and won't until NOV 15th... So angry at her and this system!

I called the union and met with somebody personally, who looked at my days and then worked it all out for me on a calendar. She then gave me the forms with post-its on it with when everything had to be submitted (before baby, after baby....). I found this very helpful especially since my payroll secretary was giving me WRONG info. When I came back, my payroll secretary tried to with hold my check and I had to call payroll and get it all sorted out. It is definitely not fun at all!!!
