Finally have a diagnosis
After 3 miscarriages in the last year, I finally have a diagnosis. I saw Dr. Alex Kofinas and had the most thorough examination and testing. Turns out I have thrombophilia, Homocystemia, MFTHR, and elevated natural killer cells. I will still see my RE, Dr. Palter to get pregnant (hopefully with IUI) starting my next cycle and my protocol is: Metanx, Lovenox, Metformin for PCOS, Prednisone and Intralipid infusions. That plus lots of supplements. It all seems so overwhelming between all the drs visits and the expense (GHI only covers some of this) but hopefully seeing both drs will bring me a healthy baby.

Dr. Kofinas is wonderful!
I became a patient of his after losing my twins due to an incompetent cervix at 21 weeks, a failed FET and a chemical pregnancy..I am now 30 weeks pregnant with a little boy. You are in great hands! Wishing you lots of luck!

Did Palter refer you to Kofinas? I'm curious bc he wasn't keen on testing for immune issues at all when I saw him last year. Wondering if he's changing his protocol.

Actually, Dr. Palter did not refer me to Kofinas. After the MCs, I was concerned that we were missing something and I decided to take the advice of one of my best friends ( who swears by Dr. Kofinas). Hopefully, the combination will bring me baby #2!

Thats great you have a diagnosis! I was first labeled as unexplained and I wanted to know what was wrong. An immune issue was found and I was on Prednisone and Intralipids. That was my successful cycle. Best of luck!
Question for you, would Kofinas treat you with those meds if you weren't working with a RE and just doing timed intercourse?

Yes. Dr. Kofinas asked what protocol Dr. Palter recommended for me. My ovulation is irregular so Dr. P feels iui is a good route for now.

I'm surprised Palter is okay with immune therapy and working with Kofins. He was pretty adamant against any immune testing and treatment last year. This is awesome though! He must be changing his views and I may consider him if I TTC again. Having Andy there in office is really convincing me.

So after careful review of Dr. Kofinas' diagnosis, Dr. Palter feels that a lot of the medications that were prescribed for me will not change my outcome and he still stands by his initial diagnosis that as we age our egg quality can diminish (I'm almost 39.) IVF with PGS is recommended as Plan A but due to my insurance only covering 75% of IVF after 3 iuis, we have decided to try iui first- it worked when I got pg with my DD almost 3 years ago. Dr. Palter does feels that this is definitely a good plan B! Dr. Palter feels that if every woman were tested as thoroughly as Dr. Kofinas does, we would all possibly come up positive for something. I have been confused about this for some time... I was relieved to get a diagnosis from Dr. Kofinas but concerned about a few things: he diagnoses almost all of his patients with thrombophilia, the treatment protocol is aggressive with lots of medication and the expense of it all. If the egg that gets fertilized is chromosomally abnormal, then all that other medication (Lovenox, intralipid infusions, Prenisone, Metformin, baby aspirin, etc) will not help. Plus I had a very healthy, wonderful pregnancy with my DD. Since trying for baby #2, I have gotten pg on my own all 3 times. Now it's time to try a monitored cycle and see how it goes. Wish me luck... Sorry for the extremely long post.

Basically this is what he told me. He's not keen on immune drugs. It's very controversial to many REs. I would consider Kofinas' findings. I know the above poster will give you great info from her experience!
