A WWYD about pediatrician screw up, or maybe mine, or both (not sure what to title this)...

Dd is turning 2 so I called to schedule her 2 yr check up and the receptionist said

"so you want to schedule her 1year visit? Her last check up was at nine months".

Me: what?! No she was definitely seen for a 1 year well check. Had her shots, blood work, etc.

After insisting like eight times, she checked "another system" and said you're right she did have her one year, but she missed her 15 and 18 month well visits....

And she did! I had no clue that she was supposed to be seen. I mean, she WAS seen at both 15 months AND 18 months exactly (plus a few in between) for sick visits, but I had no idea she was supposed to be seen for well visits at those points.

It matters mostly because of the vaccinations, which I can't understand how I was never told she missed shots...

So the receptionist said "we will schedule her now she has alt of catching up to do".

I do believe in most vaccines, BUT not giving too many too close together.

So I blame myself because, I do have a DD who is two years older, so I guess I should have known, BUT I didn't remember the vaccine schedule AT ALL!

And in my own defense, I had another baby two weeks after DD turned 1, so then I immediately started with his newborn/infant well visits, which were every month and then specialists for him...

Never did anyone mention that she needed shots.

The dr they see is only there two days a week (which I've had a problem with and should have switched a while ago), so I blame myself for that.

The receptionist made me even more angry because, she actually put it on me, joking that I must be getting old, etc.

So, I do want to change doctors and there is one in the practice who is full time who I trust, but I feel funny about switching within the same practice.

I know it comes down to my kids' health, but I don't ever want to be in apposition where the new dr isn't available, so we have to see her..

Am I making too big a deal about this? Right now she has an appointment for next week with her dr, but I don't even want to see her for that.. I swear, I question whether they even know which shots she got...

I do have her vaccination records and will be bringing them with me.

I would be very upset & switch ped's. I think it is there job to have at least called you to follow up as to why she was not coming in for her vaccainations especially since you were going to the office with the newborn someone should of made a note & followed up I am not saying the doctor but someone in the office staff.

I find it strange that her ped never said anything when she was in for the sick visits. My Dr always goes over what shots she will be getting next visit and I always make the well visits months in advance and get a call a few days before to remind me. I would be upset with the office for not really good follow up. This is why I went with a one Dr practice because I knew he would always be seeing her and would be the one to follow up. If you feel comfortable with the practice then I would absolutely switch to the full time Dr but then again I myself would probably go to a whole new practice but that is just me. For what it is worth I really don't think you should blame yourself. She will catch up on her vaccines.

I think that is crazy. At some point someone should have noticed that she was so far behind on her shots/visits! Either the doc during a sick visit or the office person when she was checking you in/out. It doesn't give me a lot of confidence that they have been paying close attention to your records. Who knows what else they could be missing!

I wouldn't be upset at the Dr. My Drs office does not call and say hey they need an appt. They gave be a shot schedule at my first appt when DS was born and it is my responsiblity to make the appts. Even at sick visits they do not say hhey don't forget to make the well visits.

As for catchign up on shots...they will not give more in a visit to make up. Your DD will just be behind. Its not that big of a deal. DS is behind because my Dr will not give shots if DS has a cold. As for switching in the practice...I don't even look at that as switching Peds since there are 5 drs at my Ped office and DS has seen them all. there are Drs I perfer but I hae to be ok with all of them sicne for sick visits you get whoever is available.

It's sad to say but this doesn't surprise me. Not that you would necessarily know this but I do think it unfortunately falls on us to make sure they're getting in for their visits. I know some dental offices send out cards telling pts that they're due for their check-ups but I don't know of any medical doctors who do this.

I know I have never been reminded to make my kids apts, although I would appreciate it if they did! It seems that they don't review the chart very thoroughly at sick visits and since they don't send out reminders, kids are going to fall through the cracks if the parents forget or don't realize they're due for well visits. I would really complain to the doctor about this, maybe they need to consider sending out reminders. I'm sure going ahead you will not have this happen again, unfortunately you learned the hard way. This sounds like a crappy system to me. I don't know if switching dr's in the office would really change anything but go with your gut if you don't like who you're currently seeing.

Thanks.

So when she Did go in for sick visits at 15 and 18 months, she was seen by different doctors within the practice, because hers is only there two days/ wk. and they probably weren't looking at the vaccines.

Her doctor definitely should have mentioned at twelve months that she has to be seen at fifteen... I don't remember her telling me and if she did, I usually would go straight to the front desk to make it. They always call to remind/confirm the day before a visit, so I know I didn't make it and miss it.

I would be bothered with their poor record keeping. I would not be bothered that they did not tell you about well visits. I don't think it is their job to remind you to make an appointment.

I don't think it is a big deal to get the vaccinations now. You can spread them out. You definitely need to get caught up by the time your child starts preschool though (assuming 3).

My dr actually does send out reminder cards and I wouldn't expect it for babies who are seen every month, or few months, but how would you KNOW when you are supposed to make an appointment if its your first kid if the dr doesn't mention it?

I know mine and my best friend's kids drs have their patients on different schedules for the first year. My first dd had a different ped in that same practice and he always told me the next time I had to bring her in.

She is starting preschool in September.

Of course they have a responsibility to tell parents when the next visit should be.

Especially, for example, if it is your first kid, or first kid with that dr., how would you know what shots they should get and when?

Again, not every dr does it the same way.

I agree that it's strange. Even if it's not your first kid, they should tell you at each well visit when to schedule the next one for. At her 15 and 18 month appts I find it surprising that they didn't glance at her records closely enough to see that she hadn't been in since a year and mention it.

I think I would consider a new practice all together. I guess I don't know what's normal, but I would hope that the office staff would be on top of it as well as the doctors.

Re: A WWYD about pediatrician screw up, or maybe mine, or both (not sure what to title this)...

DS ped checks his chart via computer every time we are there. At the end of the visit he states when he wants to see him again. If DS goes for a sick visit he usually doesn't say when his next well visit should be. I do feel that after your childs 1 yr visit the doc should have said ok see you in ? months. I don't think you are to blame per se, but I would think that after already having 1 child you should know there are other shots that are needed but I can understand that with 2 kids you may not be thinking of it. That said I do think your ped (or a new ped) will have to spread out the shots that your child missed.

My peds office never called to tell me, but the ped would look at my girls chart and say your next visit is at x months, or if were there for sick visits the ped would say your child is due for x visit or x shot make sure you make a well appt for that.

That's nice. I have to say, it is their practice to let us know At the well visit, when the NEXT well visit should be and tell us to make an appointment up front, which is what I do.

This exactly. My ped gave us a schedule at our first appointment and there's one posted in every room, so I always glance at it when we're there. At each well visit, I make the appointment for the next one so I don't forget, otherwise I'd never remember.

DD missed a shot once because she was sick. I then brought her in because I thought she had pink eye, but she didn't. While I was there I told the ped she had missed her last shot and if we could make it up that day I'd like to. He reviewed her records and cleared her for it. I didn't expect them to remind me she needed her shot. Plus, so many parents now follow a delayed schedule or don't do them at all and you can't expect them to keep up with each family in the practice.

Our ped tells us at the end of the visit when they would like to see us again.

It surprises me though since she's been there and they had her file that it didn't jump out that she missed a couple of visits (you think they would look to see what her weight was especially if they were giving her some sort of medication!)

As far as saying when to come for the next visit at the end of the well apts, then yes, my ped usually does this. I can remember though once having to say "When is she due for the next apt again?" Of course they *should* tell you when to come back but they are human and can forget. Were you under the assumption that the next apt wasn't until 2 years?

This is exactly what i thought...and why at her 12 months visit didnt they schdule her 15 month visit at that time...my office always schedules the next well visit even if its 6 months in advance, you can always change it when the time comes but at least its there

But at least they provide you with a SCHEDULE. I did not know/ remember from my first dd what the schedule was.

The way MY peds office usually does it is to remind us at the end of each well visit when to schedule the next one. So I have relied on that...

By no means do I expect a personal phone call to remind me every time. Just something (schedule, mention it at we'll visit) to guide parents when we should be bringing them in.

I wasn't completely blaming them either... Maybe I should have asked when she didn't mention it at the twelve month visit... I will attribute that to my almost nine month pregnant brain.

Yes for some reason I have no recollection of when I brought dd 1 (who had a different dr) between the age of one and two.

I was sure that her next well visit was at two.. She's probably been seen for sick visits 6 times between then.
