We didn't, we had just our family over (20 people)for a big dinner, some cake and goodies. We decorated and made it really cute.

I wish we hadn't of had a party for Lauren. It was  and out of the 50+ we invited only 8 people from DH's side came. Not one person from my family came. I felt bad for DD. We are only having a small one at our house with DH, me and the inlaws when the next DC turns 1

I did not do a "big" party for DD's first b-day. She is a December baby and I just couldn't see renting a place, catering etc etc.
Immediate family that could not be excluded totaled 20 (including DH & I). We invited 20 more, had family come two hours before everyone else, cooked pasta ourselves, served with bread and salad and soda. Had cake (two kinds) - and stuffed everyone inside our condo (not easy).
The only thing I would have done differently is made sure the video camera was on before we brought out the cake and starting singing Happy Birthday.
ETA: We had no entertainment other than the TV, no favor bags, no piñata etc. Just lots of love in the room.
