Need help/advice- this is LONG so I apologize but needed to write/vent ask for advice!

I'm 41- at is IVF thing 3 years now.

History

2008- 34 years old had benign ovarian mass- large so had it removed- twisted around ovary so lost right ovary. Single not thinking about kids etc

2012- 38 years old in serious relationship - another ovarian mass- smaller than first one but worried. Unusual issue as per dr. Could monitor it but chose Surgery- seeing fertility dr first to freeze embryos in case I lose is ovary- did two rounds froze 4 embryos. Had surgery. Dr saved ovary and removed mass. Minimal loss of ovary maybe a sliver. This was not 2012 after sandy

Fast forward to May 2013- ready to start trying for kids via fertility dr- sandy hit hard- stressful time for us- tried naturally didn't work. Start IVF process

2013 Did 2 IVFS with dr trivax- didn't work- had water sono dr sung said I was cramping and that was issue for test but said didn't see anything- no addl tests were given.

11/2013- consult w RMA- ordered bloodwork, HSG test- low and behold. Thyroid issue put on synthroid and HSG shows uterine septum. Had surgery in 12/2013 to fix-

Early 2014- partial IVF retrieval. Very fast cycle and high dose of meds had 2 eggs None of eggs fertilized but dr over medicated me.

Spring 2014. Another IVF attempt. For 3 eggs one made it to day 5- negative.

Summer 2014 decided to use my frozen embryos from 2012- wanted to keep them on reserve in case but after few failed cycles tapped into those. 2 made it to day 5 one beautiful embie- chemical pregnancy - no more left

Late Fall 2014- spring 2015 consult w dr t sirm- did immune bloodwork. Elevated NK cells husband and I partial dq alpha match - put On metanx for folic acid. Had hysteroscopy to remove scar tissue, diagnosed w hashimotos thyroid disease. Increase

Spring 2015. Started IVF cycle. Dr noticed something on soon that prev dr thought was left over follicle cyst but was there on several scans. Says I should consult gyno to get an MRI to see if it's another mass. Did IVF cycle. Long cycle produced 5 eggs. Most I ever had from only ovary. 3 were mature only one fertilized. Didn't make it past day 3

Now- went for that mri. Results came back small ovarian mass on ovary. Possible 4 mm polyp in endometrial cavity and likely tubal blockage! Went back to my hsg of 2013 and it was noted there as well. I was focused on uterine septum I didn't notice that. Two years my only tube leading to only ovary is blocked!

If you read all this thank you. I'm upset writing this. So here are my options as per dr:

* leave mass alone do IVF but get mri in 4 months to watch it. Last 2 masses were benign so likely this is benign. Get hysteroscopy to remove polyp before any transfer. Would do a frozen transfer if I get any eggs that fertilize. Better for my body.


* remove mass and ovary since this is an ongoing issue- no chance at own kids. Move to donor eggs.

* laparoscopic surgery to fix tube- odds of it working after it are 50/50. But I'm the what if type? Called gyno to call me back to discuss this option. Do one more IVF with own eggs but also try naturally as well. If IVF doesn't work and I don't get any of my own eggs to fertilize- then move to donor eggs and get surgery to remove ovary and mass.

Am I beating a dead horse? I'm going to be 42 in sept. I've been fighting this for 3 years and like my dr said you're body has been through trauma with all of these issues. Donor eggs are so $$$$$ - my younger sister offered to help but don't think I can go through with that.

Thing is I'm getting my period any minute which I was then going to start IVF- I'm teacher. Off summer perfect month to do. I can't do next month as it falls right into the beginning of school- so that means late September start

After reading this long novel, any advice?????

This is my opinion and it's coming from a person with no functioning tubes. I would probably go for the laparoscopic surgery and if that didn't work or low egg quantity and quality move on to donor eggs.

This is my opinion and I've had a lap done twice and I'm DOR. I would continue low dose ivf and watch the mass. Time is not on your side, and with no guarantee that your tube would be fixed I wouldn't waste my time with a lap. I would try and get pregnant with your own eggs as long as your willing to go for it. I'm sorry your dealing with this still.

Dealing also with DOR, if I were you, I would do another IVF cycle, hopefully get some embryos to freeze. I would try at least another cycle with your own eggs. It's a tough decision bit that's what I would do.

I feel trying with my own eggs is still an option so think we're going thT route. I have this nagging issue with that tube. I've had lap surgery before not a big deal and I think I'd feel better if the was checked/fixed for peace of mind.

I would do IVF now and not worry about the tube - you don't need it for IVF so why worry about it now?

If you are doing IVF skip anything with tube. You don't need the tube clear for IVF. Polyp in the uterus I would have removed. It's a simple procedure and the polyp can cause implantation issues.
The ovarian mass.... How is your ovarian function? Each time you touch the ovary you damage it. So removing the mass on the only one you have will hurt you.

I have a cyst on my ovary that we decided to leave alone since I an 42 and have low functioning ovaries.

Thanks ladies for input- I know I don't need the tube for IVF but that means I have no shot at getting pregnant naturally. I hear these stories about girls trying IVF then Getting pregnant on own Look at lilies. My tube has been blocked as long as I've done IVF and I'm so angry nothing was brought up and done bc that's one of major things they look for. Even with one ovary.
My friend tried IVF 3 times. Ran out of money said wth I'll try one iui bc it's covered and then she's done. She got pregnant. I just feel robbed of the possibility and I want them to fix that. And I'm curious as to how bad it is.
I'm def not having mass removed. Already had one removed and this will further damage ovary. I want to do one more go at a retrieval but this tube issue is nagging at me.

I haven't done IVF (yet) but I think I would go with option 1 (leave mass alone do IVF but get mri in 4 months to watch it.)

First I give you a lot of credit for all that you've been through. I feel like because you've come to this point and you are questioning it, maybe try IVF one more time and see what happens. Have you done acupuncture and supplements? DHEA? Has the dr given you any reason why your embies are not making it past day 3? I would want to know that too before moving forward. I wish you all the best and i really hope whatever you choose works for you and your DH! all the best!

I just want to wish you the best of luck in your next cycle! I hope you get your BFP, and that you don't have to research donor eggs, however if it comes to it feel free to message me with any questions, as we are in the midst of a donor egg cycle!

Uhhh I'm so sorry for all you have been through. I have been following your story as long as I've been on here. You have really tried so many different things. I want to know why they think the embryo is not sticking when you had the transfers. Did you get the EFT test with Dr. Tortoriello? I was going to get that if the IVF didn't work with him. Since he will be treating you for immune issues I think you have a better chance. I know you had a chemical so the embryo was trying to stick and didn't stay for whatever reason. The immune meds (prednisone and intralipids) definitely helped me. We transferred 2 and 1 stuck. So if I was you I would give IVF with your own eggs one more shot. I hope that works and if not then I would look into donor eggs. I hope this is it for you! You are such a strong woman.

Bank whatever frosties you're willing to pursue (stress, $$, etc.) now while keeping an eye on that mass and have your insides cleaned up before attempting FET.
