DS got mostly toys and money in April. He got a few outfits but his grandmas and aunts are always coming over with bags of clothes for him so we never really need clothes.
If people asked, I told them pretty much to get what they wanted, he really didn't need anything. I was worried about being overrun with toys but after hearing my in-laws complain that we asked them to get him a toy box from land of nod at Christmas and not just a ridiculous amount of toys, we decided that we would just let people do what they want and if we needed to donate or return things we would. Luckily it was a nice mix of things and we haven't needed to purchase a toy for him since! He's just as happy playing in the cabinets anyway.

Most people asked what we needed for the girls thankfully. Most gave money and gift cards since they didn't know what size to get on clothing. I've always told those that ask that learning toys and books are always great.

I've given everything from clothing to Disney stock. Walking toys are pretty common too.

We've had people ask - it's mostly family - so DH sent suggestions of age appropriate toys and the size of clothes that he wears.
I like to try to get a special gift for first birthdays, so in the past we've given to our nephews and neices the complete Beatrix Potter story set, jewellry, a silverplated piggy bank.

you just reminded me, in answer to your thread, we are getting Luce for his first birthday the complete dr. seuss collection as his gift, beside all the travel stuff.

If it makes you feel any better, Colin has been sleeping in Ryan's RECENTLY old onesies - 24 months  They literally share clothes some days 
To answer your questions. Def. toys (you likely don't need  ) and clothes. I try to assign grandparents/family members the bigger items (when they ask for ideas) so I don't end up with duplicates. Ryan's first bday ended up in two cars - annoying to return. I try to think ahead with toys and clothes.
And gear - nothing wrong with suggesting that for a gift (a booster seat you may not have but will need soon enough, etc.)

I did a wish list and EVERYONE of my guest at the party thanked me for it - Id idnt go crazy and put on all prices too - I also included in the notes the size DD was for the summer if clothes were to be purchased.... For us I did it because I was getting a million phone calls about what to get.. LOL...
Also, I got almost all the girt receipts too so exchanges were nice and easy....
Anyway - Good luck on what you decide...

I personally buy clothes for most kids 4 and under, and leave the toys for Christmas and parents. I do this for a two reasons . . . first I don't want to buy a toy that a parent would object to (or that the child already has) . . . second at such a young age they grow so fast that clothes are a HUGE expense for parents, and IMHO, a gift of clothes can help a tiny bit. If I know a parent doesn't want clothes (like two friends I have - one who is a clothes horse and buys out the stores all the time, and the other who has enough hand-me-downs to last her child until he is in HS), I then specifically ask what their child would like.

Most of DS's family (aunts, uncles, grandparents, etc) gave money. We got a couple of outfits and a bunch of toys.
I wouldn't wait on the clothes that you get gifted because sometimes I find them to not be the nicest of outfits.
And he wound up with so many toys that I knew he wouldn't play with. I tried bringing them back to TRU but not many people gave gift receipts. So TRU wouldn't take them back.
