Wipes get one of those flat travel cases
Paci with case
I keep bathroom garbage size garbage bags (i get at dollar tree) in case we r out and go #2 ( i also leave some on changing table since i dont use diaper pail)
Cream or a/d type stuff what ever u prefer using

In addition to what everyone else said...
bottles/formula for however long we're going to be out for
a change of clothes
1 or 2 receiving blankets
burp clothes
a brush
some vaseline or butt paste

EXTRA CLOTHES. DD is 7 weeks and every time we visit family/friends she seems to have an expolsive diaper which ruins her outfit.
Also, wipes, diapers, plastic bags (for dirty diapers and if LO does ruin clothes you'll need soemwhere to put them until you get home), pacifiers, pacifier wipes, I BF but I always have a few Similac powder singles in the bag with a bottle in case of an emergency, burp cloth, cover-up if you're BFing, Purell, changing pad.

DD is 9 months now - I like to have a few toys for her (small like rattles, teething toys), a nursing cover for me, I have children's tylenol, tylenol for me, little samples of eucerin and aveeno (from the doc's office), hand wipes (for me) and a bottle of water.
This is in addition to what others mentioned. And during the colder months I kept socks, infant mittens and a little hat in there for DD. If you nurse you may also want to keep lansinoh and breast pads for yourself, especially in the beginning. I also keep a change of clothes (for me) in the car in case a poop explosion from DD hits me!

this is what I always had when dd was a newborn:
diapers
wipes
aquafor
disposable bags (for dirty diapers and soiled clothes)
2 burp cloths
2 bibs
purell
extra pacifier
extra clothes
receiving blanket (especially when in the ped office because they have you strip the baby down to the diaper and it's cold in there)
extra bottle and formula
mylicon
a bottle of water and snacks for me (I used keep a couple of granola bars in the bag)

an extra set of clothes
a burp cloth
paci
wipes
extra diapers

a bib
infant tylenol/motrin
bulb aspirator
hand sanitizer
a few little rattles/toys
receiving blanket
baby ambisol for when they teethe
mylicon drops for gas
a comb or brush
ETA: the bulb aspirator seems silly to carry around but I will tell you it is SO important to have one with you all the time. DS used to spit up a lot as a newborn and several times it must have went up his nose. The aspirator came in handy!!!
