Questions for RE after failed IVF
Hi all Yesterday we got a BFN on our first IVF cycle. Obviously DH and I are devastated but we knew going into it that it wasn't a guaranteed positive. We went with IVF because DH has a low count, motility and morphology. We transferred 3 embryos on day 3 only one of those was 8 cell. So my question to you is, we have an appointment today with my RE and I'm trying to come up with a list of questions to ask him. Have any of you been through this and what would you ask? TIA!

I would say what can we do different and is there anything else I can be tested for . Also you want to know what can be done to improve the quality of your eggs or embryos . After my ivf , I realized it was time to switch re's. When I got a bfn after putting in two perfect embryos my re response was lets do another ivf . Not good enough for me. So I knew it was time to move on to another dr .

Doctor Q (based on my failed IVFs)
Many RE are transferring on Day 5 not 3 anymore. What's your reason for transferring at Day 3?
Any additional tests that I need to get done that I haven't (HSG, Saline Sono, endometrial biopsy, immune testing (only certain drs. do this)
Would you change dose in medicine? any prednisone? (dr. added that after first two failed- lowers any inflammation)

I'm sorry it didn't work.
In addition to the other suggestions, I'd also ask their thoughts about why you only had one 8 celled embryo. How did you body respond to the meds? Were all of your eggs mature? How many follicles did you have? Sometimes looking at how your body responded to the protocol can give them insight into what they can do to improve your next cycle.

So sorry your cycle didn't work. Os your DH on medication and working with a urologist to try and improve his sperm? If not I would suggest discussing this. I also have been down the path where I was told it was male factor and in fact I too had issues. I would make sure you are FULLY tested before doing another round. I would ask why your re thinks only one had 8 cells. Good luck.

like other posted why day 3? what would he change as far as protocol what can your husband take to help with motility good luck and so sorry for the results

Thank you all for your response! We had a very positive visit with my RE yesterday, not changing much of my protocol because I responded so well the first time, doing some things to improve egg quality and adding a supplement to my DH's vitamins. The only other thing he is changing is Crinone twice a day rather than once and adding estrogen. The reason for the day 3 transfer was that only 4 eggs fertilized and only 3 were making it to day 3 at that point, 2 weren't going to make it to day 5 and he didn't want to take the chance on losing the 3rd so he and the embryologist decided on a day 3 transfer. This time around we are hoping and praying for more eggs to fertilize and make to day 5 and freeze.

With me my new re started me on supplements at our first visit so I was on Dhea and coq10 for 2-3 mos prior. The. He also upped my dose of meds as the amount I was in prior was not enough. He also added in a human growth hormone . Not all re will do this as it is not Fda approved for ivf but there is a lot of positive research. It helped the quality . Needless to say not sure what change caused the difference . In the end although I only wound up with one perfect embryo on day three it worked as I'm 23 weeks . Some re don't believe in all of this out of the box stuff which can really make a difference .

He is adding DHEA and CQ10 for me this time around as well. Also on a high protein low carb diet.

Glad you had a good appointment. We had similar issues with number of eggs, fertilizing etc and my RE said and did the same thing about the 3 day transfer. The way my RE explained it, embryos do best in your uterus so if you have a low number of embies and wait until day 5, you might get none lasting but some of them might have done ok if transferred on day 3. Waiting until Day 5 gives you the most info about embryo quality but with a low number a Day 3 transfer gives you the best chances.
