Child Health Plus
Anyone have Child Health Plus for their child/children? I'm trying to fill out the application and it's confusing me!

they have places you go and they help you fill it out
are you in Nassau?
QLIMG in Hicksville has people there
if you call the 1800# they will schedule you an appt and you go through everything in like 20 minutes and they mail everything out for you
