Wow some of these prices are outrageous for a measly piece of paper! The only policy my ped has on forms is that it can take up to a week to fill it out due to the high volume of forms which I can understand. For the form I needed for DD's nursery school, I dropped it off on a Tuesday and they called me the next day to pick it up.

my ped. will provide their own form for free...if you ask for a form your provide to be filled out they charge $10.

Never and they have always updated both kids forms everytime we go for shots as I need it for daycare

Seriously and I would think it's part of the cost of doing business. I mean, they are MY medical records for MY child which they obviously won't give me a copy of either without paying a fee. It's not the $5, it's more of the principle of it I guess.
DD's ped. doesn't charge anything.

Mine didn't for daycare forms, but they needed a few days notice.
I am shocked at some of the prices, I mean, it isn't exactly the same, but HIPPA only allowed them to charge .75 a page for record reproduction..

I work at a peds office, and I have NEVER heard of charging to fill out forms. To me, that is CRAZY!!
