Either infant car seat in the wagon and groceries around it, or I brought the stroller if I only needed a few things. Most of the time option 1.

When they were in the car seat, I'd use the snap n go and put the groceries underneath. Or wear them.
When they were prob 6months on, they'd go in the front of the cart with a cushion floppy seat cover.

I shopped more often (2 -3 times a week) and put groceries under my stroller. I have a BIG basket under it and found I can fit more their than in the cart with the car seat in it. It gets much easier once they can sit in stroller. Large items like paper towel rolls I'd buy on separate trips i.e., CVS and just carry them.

With baby in stroller, I used a handbasket. If baby was big enough and I needed a cart, I would wear baby.
And, DH helped a lot with shopping or picking up take out.

Easiest to me was to wear them. Thankfully it's a pretty short phase before they are old enough to sit in the cart. I did wear my younger one until she was probably 1.5 if I had both girls with me so I could have my older one sit in the cart.
ETA I also used shoprite store pick up plenty of times. No getting out of the car so nice and easy.

I used to wear my daughter when I went shopping-I miss being able to wear her now she sits in the shopping cart and asks for tons of stuff

Before she was able to sit in the cart herself (around 5 months), I would take the stroller in and just fit whatever I could in the stroller basket. I also carried a reuseable bag and stuffed things in there.

Definitely use a baby carrier and wear baby. Baby is all snuggled and cozy and you have the shopping cart free for groceries (the car seat would take up most of the cart and a stroller basket is way too small...) If you're nursing you can even nurse baby in the carrier!

Wear her. Although your shopping will take triple the time because every old lady in the store will stop you to coo and try to touch you baby
