$200 a month
We are very lucky. We have great medical, dental & vision through DH's job.
Our co-pays are $5, our prescriptions are $2, we each have $3,000/yr in dental coverage & $200/yr for vision.
I recently had fertility treatments & ins pd for everything. I paid just the co-pay - total of $75 for all visits.
Everyone should have this good of coverage IMO.

through my employer: $74/mo for my medical (one of the better plans offered --- I need it, although it is an HMO). could have been as low as $0 for a high deductible plan. Was $260/mo when my DH was also on my plan but he is now covered through his job, because he doesn't need the same bells and whistles I do.
also pay an extra $40/mo toward health insurance for being a "high-income employee" (which my employer just implemented in 2010)
$6/mo or so for vision; $17/mo or so for dental (for me only, not sure what DH's dental costs but I think it's similar)
FSA account and subsidized prescriptions

We pay 450/month for medical for family. Dental and vision is another 50/month.
At my last job, I paid almost $1100/month for medical and $60/month for dental. It was a PPO plan with a $250 deductible. The whole time I was there, I really hoped that they would raise the deductible to reduce our monthly premium.

$68 per month. My employer provides excellent coverage. That includes a PPO plan, dental and vision.
FH has some type of crappy "discount" plan called "Perfect Health." It's comprised of basically just healthcare savings plan that his employe deposits a set amount of $$ into and that covers DR visits, etc. Then a discounted PX card. In one shot when FH fell, the account was wiped out until the next time the employer put $ in. The PX's that FH takes (Advair, Albuterol, etc) are NOT cheap and takes a chunk out of the account too.
I can't wait till we get married and I put him on mine.

I believe about $7 or $8 a month thru DH's job.
We have $5 co-pays for prescriptions and $15 co-pays for doctor's visits.
I never EVER complain about our health coverage---it's amazing and I know we are very fortunate.

we have a high deductible plan and have to pay out $6k before the insurance kicks in (except for child well visits and ONE ob/gyn apt for me) each year. it covers dh/moi/ds
not sure how much addt $$ his firm pays

$1250 a month. $15,000 a year! My dh's base salary goes largely to insurance.
ETA: no dental or vision. We pay separately for a discount dental plan.

$30 per check (every 2 weeks) for DH and me. $20 co-pays at any office visit. Meds are either $10, $25, $50 or full price. No dental, no vision.

we pay nothing for our coverage. through DH
our copays for doctor visits are $5. and yearly deductible is $50
i am sooo thankful for our insurance plan.. especially after doing fertility treatments and now seeing a doctor that is only out of network
