Thick Nuchal fold- UPDATED 6/7/12
I am 22 weeks and had an ultrasound done on Monday. The tech said everything looked great, but today I reviewed the results with my doctor and he said that the nuchal fold was thick which is a marker for downs syndrome and that was the only marker they found. I am a mess, has this happened to anyone and then gave birth to a healthy baby who does not have DS. I had the NT testing done at 12 or 13 weeks and everything was fine.

That doesn't make sense to me. It has always been my understanding that the ultrascreen is specifically done between 11 and 13 weeks because after that, the measurements of soft markers for DS (like the nuchal fold) and other chromosomal abnormalities isn't as reliable.
Obviously I am not a doctor but I'd definitely get a second opinion. Good luck

I thought that they test that at 11- 13 weeks as well as they do blood work. I would definitely get another opinion. 22 weeks sounds more like a level 2. I would think most everything is bigger then.

I don't even know why they would bring that up now? I was told they can't measure the nuchal folds after 13 weeks because they aren't transparent enough. I would not worry about this at all

I had it done at 12-13 weeks already. Maybe there is a difference in what they test for between 13-22 weeks. I don't really know. But I am a complete mess.

Did they find anything at the NT at 12-13 weeks? Is what you had the other day the anatomy scan? What did he say your next steps should be? I would make a list of question and give him a call to clarify and see what you should do.

Everything was fine at the 12-13 week testing. I spoke to the genetic counselor and she said everything was good. I went for AFP testing and my Dr. suggested another sono and said that some further testing I can do are the amnio or the CVS. I don't know what to do, or how in the world I am going to function at work today. The last thing I want to do is listen to other people's problems when all I want to do is cry.

I know it's hard, but I would try not to go crazy. The NF thickness is only one marker for DS and alone it doesn't mean much. I think there is usually more concern when there are several markers. If your anatomy scan otherwise was fine than but some weight in that.
I know they gave me odds of DS when I had the screening at 12-13 weeks. Since you said that it was fine at that screening I'm sure the odds were low, did they give you new odds at the most recent sonogram? My husband and I discussed this before our 12-13 week scan with out first, that if the odds of DS were higher than than the odds of having a miscarriage via an Amnio I would go ahead with the Amnio. We asked our doctor what the chances were, he gave us some number I can't recall now.
Did you doctor advise and amnio or a CVS? Did he counsel you at all about your next steps or refer you to a genetic counselor?? If he didn't do any of those things I'd likely find a new doctor. It seems reckless to me to give you this information, cause you to worry, with no real advice for the next step?

i get what you are saying. who knows. maybe the doc wasnt in to speak with the couple and what would she say "something looks off but the doctor isnt in, ill have him call you tomorrow!" I would have freaked and stayed put until I spoke with someone. who knows, maybe the tech didn't even see anything and the doc picked up on it after reviewing the test. so many what ifs.
To the OP - I am sure everything will be OK! If the NT looked good, perhaps the measurments were just off or the machine needs calibration

So sorry that you're going through this I also thought that the u/s at 11-13 weeks for the the testing of the fluid; and then if there was a problem with that test they send you for testing further along.

This is why I have such an issue with all this testing they do today. This is making you crazy and I'm telling you it will be for nothing. They scared the life out of my sister with both of her pregnancies and everything was healthy and beautiful with both! I agree with everyone else that those results should have already been done 10 weeks ago, which they were and everything was great. ****Also, I would research getting an amnio or CVS at this point in your pregnancy. I believe your odds of miscarrying or going into pre-term labor are greater at this stage of pregnancy.

I was told at my 12 week NT scan that he had an enlarged nuchal fold. In terms of numbers, they said he was a 4.7 and the absolute max they would want is a 3.5... A 2.5 is average. My baby's was nearly double the average!
I won't lie. I went through the next 12 weeks after that in a state of pure torture not knowing what was wrong.
I had the CVS testing done, which is usually done by 12-13 weeks because they sample the placenta, not the amniotic fluid. At 22 weeks, I wouldn't opt for a CVS. Possibly an amnio, but with no other markers, I think you would be fine.
The CVS testing came back fine! all chromosomes normal. I found relief in that, and knew since then he was a boy I then had early anatomy scans, a more detailed Level 2 and a fetal echocardiogram, as well as genetic counseling and everything was fine. He was born happy and healthy.
I know it's hard and I did not take my own advice, but stay off the internet! It will drive you crazy!
If you have any questions, FM me. I do know that every sono scan I had after my 12 weeks scan, I was told that the nuchal fold was moving more toward normal. So they do measure it with each sono. (at least in my case, they did).

Awwwwww that IS a gorgeous newborn!!!! Too sweet.
To the OP: I agree with what the other girls are saying. The whole thing truly sounds off to me. The NT test is done at 11-13 weeks for a reason and if that came back good then I don't know why they're remeasuring baby's nuchal fold. It's bizarre. Believe me, I'd be freaking out just as you are because you're a good mommy worrying about her baby. But I honestly think you should get a second opinion. Like I said, it seems a bit off to me.

Thank you to everyone for their posts. I called the genetic counselor today at Madonna perinatal and thankfully they were able to see me this morning. They had me do the quad test, she said that I am a few days past the testing time, so the lab might reject the test, but they were going to try and send it out anyway. The genetic counselor said that the maximum NF thickness is 6mm and LO was 6.2. Not really big in the sense of numbers, but to an expectant mother that number is enough. I had them do another sono this afternoon and the nuchal fold came back completely normal. It went from 6.2 and is now 4.3. I am not sure if the 6.2 reading was from miscalcuation by the first sono tech or because of the way the baby was positioned which is what the Dr. and more experienced sono tech at Madonna Perinatal believes. The one thing I was able to think of was during the first sono the machine kept freezing and the baby would not stop moving and she was having a hard time measuring everything. I still have to see the pediatric cardiologist to schedule the echo based on the original sono, but at least I can breathe now. I see Dr. Sampino and Vullo and have COMPLETE faith in them and I don't doubt their skills for a second, I declined both the CVS and the Amnio with them and opted to see the genetic counselor and complete whatever bloodwork was suggested or necessary.
