i cook for us - one of the reasons i do too is bc he works 2 jobs so he comes home from one eats and then 30 mins later out the dorr to his other job so if i dont cook he wont be able to eat.

depends, if ds has a good day then I cook... if we are having a not-so-good day, then either my husband cooks when he comes home or we just make sandwiches and salad, something simple( we dont have to have dinner every night)

I do. All the time. Or we wouldn't eat  DH works too late to have him cook dinner... On days he is home, he can't multi-task, i.e., watch the kids and cook dinner. So we might order take-out if I don't feel like cooking.

I do cook most nights and not just heat something up - although if DD is really fussy DH will just take over and cook the meal. I cook for us whatever I want and DH is happy with that - sometimes he does request fish or chicken which I usually make anyway but he never requests a certain dish. DH will watch DD while I cook so it works out okay - he gets to spend some time with her which is nice. DH cooked for me every night I was pregnant b/c I had horrible food aversions.
