Transitioning to Milk from formula?

Transitioning to Milk from formula?
Our ped said we can start to transition at 11 months. My question is: did you mix the milk with the formula or serve it separately?
I was thinking of mixing...
week 1: 2oz milk/6oz formula
week 2: 4oz milk/4oz formula
week 3: 6oz milk/2oz formula
week 4: all milk
Also did you wean your child off the bottle at the same time or wait til they were fully on milk.

This is exactly how we did but I didn't do it for a whole week i did it for 2 days for each equation. MY DD loved it and I think likes the milk better then the formula. As for the bottle I still used nipples until about 14 months. She still drinks her morning milk from a bottle but I put a sippy attachment on it instead of a nipple. The rest of her milk is consumed in a sippy cup

We did it by decreasing the formula and increasing the milk by an eighth each day til the bottle was 100% milk. It worked out fine for us. Abby also went from soy formula to whole organic milk with no problems whatsoever. We did have a pretty full battery of allergy tests done beforehand though just to make sure.

I did cold turkey and they both didnt have a problem with it. Ds was my first and i did not think to switch to a sippy cup when gave him the milk - DD gave milk in a sippy and didnt have a problem with it.
