A third bedroom and a second floor. We may not be able to do the second floor but we plan on adding another bedroom.
Oh and a real floor in the kitchen rather than carpet.  Yes, I said carpet.

I wsh the living room on the main floor was bigger... but we also have a den (currenly studio apartment) and finished basement so I can't complain much.
ETA: More closet space!

I'm pretty happy but I think I'd just tweak a few things or finish some big projects  that we won't be able to do while 2 DC's are in daycare (large patio, portico/front porch).
Full basement instead of 1/2
Enlarge the den
Need 3 more feet in laundry area and half bath
Walk in closet in MB, we got rid of it to expand our bathroom.
Bonus room at the top of our stairs... it was an option on our model home, some have it on our block but the original owners didn't get it.

Two things:
1. Another full bath upstairs in the master bedroom.
2. An all weather room attached to the back of the house, so there was another room to go to, to get away from life in the tv room.

A Master Suite... a little hideway for me and Rob.
Also - a much bigger kitchen with better cabinetry.
And - since we are going for broke here - I think I would really appreciate bumping out the back of the house so that the work out room is bigger and our main floor bath is larger.
But - my house is in good repair and has everything we NEED.. so I am happy even without these things!

A real master bath- we have an entrance to the bathroom from our bedroom which has made me wish we had a private master bathroom.

- A bathroom downstairs (Our both bathrooms are upstairs)
- I wish I could put the washer and dryer upstairs..That would be AWESOME.
- A gorgeous In Ground Pool
- An updated bathroom. The original bathroom is still here.

Insulation!! 
in addition to that -
- a larger kitchen
- radiant heat
- direct access to the backyard via a back door and not just by the side door
- a larger backyard with a nice fecne
